We reject the notion of “one size fits all.” Every support plan is crafted from the ground up with the resident’s voice at the centre. The process involves:
- In‑depth assessment: We examine physical health, mental well‑being, life history, cultural background, and personal aspirations.
- Goal setting workshops: Residents work with care planners to set short‑term and long‑term objectives—be it cooking a new dish, obtaining a job, or travelling solo.
- Dynamic monitoring: Plans are reviewed at least quarterly (or sooner if needed) and adjusted to reflect progress or changing circumstances.
Empowering Choice
Our personalised plans ensure that residents maintain agency over:
- Daily routines: Meal times, medication schedules, and social activities can be tailored to their preferences.
- Skill development: Residents select which life skills they want to focus on—be it budgeting, digital literacy, or physical fitness.
- Cultural expression: Plans respect dietary restrictions, religious observances, and language preferences, embedding them into everyday care.
